This type is not listed in the So-Called dolllar book. It is different from the type I 1902 issue (HK-649) and the type II 1903 issue (HK-650). Both the obverse and reverse are completely different from the other two types of medals from this event.
This is the only example of this type that I have ever seen, and the only example of this type in the NGC census. Unique??? Unicorn???
I purchased this medal on Ebay, thinking that it was an HK-649. Much to my surprise after it arrived and I reviewed it, the medal was quite different. I sent it to NGC for grading, not having any idea if they would grade it or not, and NGC created a new number for the type.
